====== Gastrinom ======

**Synonym: Zollinger-Ellison-Syndrom**
===== Ursache =====

  * Inselzell-Tumore bilden zu viel Gastrin → Hypersekretion von Magensäure → Gastritis bis Ulkus
  * Betroffen sind eher mittelalte bis alte Hunde

===== Symptome =====

  * DF bis Meläna
  * Erbrechen evtl. mit Blut
  * Anorexie
  * Gewichtsverlust

===== Diagnose =====

  * Blut
      * reg. Anämie
      * met. Alkalose, Hypokaliämie, Hyponatriämie, Hypochlorämie
  * Endoskopie
      * Ösophagitis
      * hypertrophe Gastritis und gastroduoenale Ulzerationen
  * Ausschluss Differentialdiagnosen
      * CNI, Pylorusstenose, chron. Gastritis, Hepatopathie, Enteropathie des Basenji
  * US Abdomen: Gastrinome sind klein und meist nicht darstellbar
  * CT ein bisschen sensitiver als US

===== Therapie =====

  * OP ist selten kurativ wegen hohen Metastasierungsrate
  * Medis: H2-Rezeptor-Antagonist, Protonenpumpen-Inhibitor

===== Prognose =====

  * bei Diagnose bei 85% schon Metastasen
  * MÜZ 18 Monate
